To our Spartan Athletic Families: Welcome to online athletic registration for SHS and SMS Athletic and Physical packets through FamilyID (www.familyid.com). All documents are to be signed and submitted online with the exception of the CHSAA Physical Form which must be completed by a medical professional. After completion, the CHSAA Physical Form must be uploaded at FamilyID.
FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible. When you register through FamilyID, the system keeps track of your information in your FamilyID profile. You enter your information only once for each family member for multiple uses and multiple programs.
If you have not used FamilyID before, create your FamilyID account and add each student athlete (7-12th grade) to your family profile. You will need only one account for your whole family.
If you already have a FamilyID account, you do not need to create a new one - just log in to the account you have. Then add or match our record doe the student athlete to the people in your family profile.
Next, complete the appropriate Salida High School or Salida Middle School Athletic Registration 2022-2023 for your student athlete.
Salida School District is very happy to offer you the convenience of online athletics registration through FamilyID. Once you submit a registration, all your information will be saved to re-use for future programs and activities. You can review your registration history and update your registration data at any time.
